The Hidden Cost Traps HR Misses When Teams Work Across Borders

Seven cost traps that never show up on the benefits invoice β€” and what they typically cost a 100-person distributed team.

The Invoice Is Not the Cost

The benefits invoice tells you what you paid for cover. It does not tell you what the cover actually cost the business. For cross-border teams, the gap between those two numbers averages 15–22% of total spend.

The seven traps below are the most common drivers of that gap, sourced from coverage audits we've seen across distributed teams of 30–500 employees.

The Seven Cost Traps

Double-coverage stacking

8–12% of spend

Global travel cover + country health + stipends + EAP often overlap. Map each benefit against each risk; cut the duplicate that's least used.

Broker commission cascade

3–6% of premium

Each country broker takes a commission. In 10 countries that's 10 commissions β€” frequently invisible because they're embedded in the premium.

Renewal-fatigue concessions

5–9% of premium

HR teams under time pressure accept renewal terms without negotiation. Each unchallenged renewal compounds.

Currency-hedge gap

2–4% of spend

Premiums billed in local currency without hedging become a year-end shock. Companies billing in GBP/USD often skip this entirely.

Statutory floor overpayment

1–3% of spend

Buying private cover that duplicates statutory minimums (UK NHS, France Mutuelle, Germany statutory) means paying twice for the same care.

Stipend tax inefficiency

Up to 35% of stipend value

Cash stipends are taxable in most jurisdictions; pre-tax benefit equivalents deliver more value per employer pound spent.

Inactive-policy creep

1–4% of spend

Policies for employees who've left or transferred continue to renew silently. Annual coverage audit recovers it.

How to Run the Audit

Build a grid: every employee on the rows, every benefit-and-country combination on the columns. For each cell, record what risk the benefit covers, the annual cost, and the utilisation rate. Overlapping cells are double-coverage. Empty cells where utilisation should be high are coverage gaps. Both cost real money.
